New Plan Year Begins
All 2025–2026 elections take effect. New carrier alert: Dental, Vision, and Life are with new carriers this year. Alliance will not administer Genomic Life after this date.

Wellness Form Deadline
New participants: 60 days to submit Physician Form + Lab Report. Existing participants: 30 days from last physical or Oct 1 — whichever is later. Physicals from 5/1/2025–7/31/2025 qualify. Submit to hrofficebenefits@alliance.us.

Every Year
FSA Elections Don't Roll Over
Health Care FSA and Dependent Care FSA elections must be actively re-elected each year in eNavigator. They do not carry forward automatically.
New employees: Register at employeenavigator.com/Benefits/Account/Register using Company Identifier ABSOFFICE and your last 4 SSN as your PIN.
Returning employees: Log in at employeenavigator.com/benefits/Account/Login
Click Start Enrollment → Enroll in Your Benefits → Get Started and step through each benefit category.
Add dependents before selecting plans. Have each dependent's full name, SSN, gender, and date of birth ready.
Review your Enrollment Summary, click Click to Sign, and confirm the screen reads "Acknowledged and Submitted."
Complete the Tobacco Attestation honestly — inaccurate answers may result in retroactive surcharge charges.

All contributions are pre-tax. The spousal surcharge ($100/month = $50 semi-monthly) applies when your spouse has access to employer-sponsored coverage elsewhere. Submit the Spousal Surcharge Affidavit to HR to waive it.

Tobacco Surcharge

Applied to tobacco users & enrolled spousesCigarettes, e-cigarettes, cigars, pipe tobacco, and chewing tobacco. Vaping is not subject to the surcharge.
How to waive
Option 1 — Non-user: Submit the Tobacco-Free Affidavit during enrollment.
Option 2 — Quit program: Complete WellRight "No Butts" + "Tobacco Cessation University" or "Health Coach" challenge within 3 months.
Free Rx help
Nicotine gum, patches, Chantix, inhalers & nasal spray — $0 copay via ProAct (max 2 × 90-day courses). Call ProAct: 877-635-9545
Spousal Surcharge
$100/mo
Applied when your spouse has coverage available at their employerApplies to medical only — not dental or vision. Deducted pre-tax.
Exempt if…
Spouse is not employed · works at ABS · covered by Medicare / Medicaid / TRICARE · on COBRA · employer doesn't offer coverage · or employer charges employee 100% of the premium.
To waive
Submit the Spousal Surcharge Affidavit to HR certifying your spouse does not have access to employer-sponsored medical coverage.
Wellness Surcharge
Applied if…
You skip the annual wellness requirementsAnnual physical + fasting labs (Lipid Panel + Glucose) + biometrics. All enrolled dependents over 18 must also complete.

Tobacco-Free Affidavit — Employee For non-tobacco users or those tobacco-free ≥ 6 months. Required at enrollment.
No Tobacco Use Affidavit — Spouse Required if your enrolled spouse is also tobacco-free. Both must sign.
Spousal Surcharge Affidavit Waives the $100/month surcharge. Certifies spouse has no access to employer coverage.
Physician Screening Form Annual wellness requirement. Submit with official lab results to hrofficebenefits@alliance.us.

Annual Physical Exam
Complete a full annual physical. Physicals dated on or after 5/31/2025 are accepted.

Fasting Lab Work
Required: Lipid Panel (Total / HDL / LDL Cholesterol + Triglycerides) and Fasting Glucose. Blood must be drawn by venipuncture — finger sticks are not accepted.

Biometrics
Your physician must record: Blood Pressure, Height, Weight, and Waist Circumference on the Physician Screening Form.

Submit Documentation
Email your completed Physician Screening Form + official lab report (must include your name, DOB, test date, and results) to hrofficebenefits@alliance.us. A physician's letter alone is not sufficient.

Your elections are locked in for the full plan year unless you experience a Qualifying Life Event (QLE). Contact HR within 30 days of the event to make a change.

Marriage, divorce, legal separation, annulment, or death of spouse
Birth, adoption, or placement of a foster child
Change in employment status — you, your spouse, or domestic partner
Change in work hours affecting benefits eligibility
Dependent losing eligibility (e.g., child turning 26)
Entitlement to Medicare or Medicaid
